Tremor is not one thing. Resting tremor appears when a limb is supported and still; action tremor appears during movement; postural tremor appears when holding a position against gravity. Which one is present is clinically meaningful, which is why structured tests beat a general impression.

A journal is useful because tremor varies with fatigue, caffeine, stress and time of day, and that variation is invisible from a single observation. What a log can provide is a trend over months, which is the thing a person cannot hold in memory and a clinician cannot see in one appointment. It is a record to bring to a professional, not a diagnosis.
